Collateral Clarity: Balloon Occlusion of an Epicardial Right Coronary Artery Branch to Map Left Anterior Descending Artery Perfusion

Video 1. Left coronary angiography in a left coronary artery RAO 24°/CRAN 3° projection showed an in-stent chronic total occlusion of the mid-left anterior descending artery with no distal opacification. 

Video 2. Complementary left coronary angiography in LAO 4°/CRAN 35° projection confirmed mid-left anterior descending artery in-stent chronic total occlusion with absent antegrade distal flow. 

Video 3. Right coronary angiography in LAO 6°/CRAN 24° projection showed a single epicardial collateral arising from the acute marginal branch and supplying the distal left anterior descending artery.  

Video 4. Right coronary artery-acute marginal epicardial collateral to the left anterior descending artery, RAO 38°/CRAN 13°. 

Video 5. Balloon occlusion of the acute marginal branch demonstrated in LAO 25°/CRAN 6° projection showed complete interruption of epicardial collateral flow without recruitment of additional channels. 

Video 6. Balloon occlusion of the acute marginal branch demonstrated in RAO 28°/CRAN 34°projection showed complete interruption of epicardial collateral flow without recruitment of additional channels. 

Video 7. Post-balloon deflation angiography showed prompt restoration of right coronary artery–acute marginal collateral flow to the distal left anterior descending artery.
