Component-Resolved Diagnostics May Enable Precision Allergen Profiling in Atopic Dermatitis

Component-resolved diagnostics (CRD) may offer a more precise approach to allergen assessment in atopic dermatitis (AD), according to a review examining its potential role in personalized disease management. The findings highlight how molecular allergen profiling could improve understanding of disease heterogeneity and guide targeted care in patients with complex sensitization patterns.

AD is characterized by barrier dysfunction, chronic inflammation, and increased susceptibility to environmental and food allergens. Many patients exhibit polysensitization, making it difficult to distinguish clinically relevant triggers using conventional testing. CRD addresses this limitation by measuring specific IgE responses to individual allergen components rather than whole extracts.

The review notes that CRD “provides additional insights, especially in polysensitized patients,” including identification of genuine sensitization vs cross-reactivity. This distinction may be particularly relevant when evaluating allergens associated with low abundance or stability, or those linked to higher risk of severe reactions. CRD may also inform the composition of allergen immunotherapy by identifying clinically meaningful targets.

Beyond diagnostic refinement, CRD may contribute to disease stratification. Molecular profiling of allergen components can help define distinct endotypes and phenotypes of AD. The authors report that this approach “provides biomarkers of disease severity, autoimmune IgE responses, and therapeutic response,” suggesting a role in predicting disease course and treatment outcomes.

Although CRD has been applied in other allergic conditions, its use in AD has not been extensively studied. The authors emphasize that this review is among the first to comprehensively evaluate its potential in this population.
