Biologic Therapy Improves Psychological Well-Being and Quality of Life in Psoriasis

Biologic therapy for moderate-to-severe plaque psoriasis was associated with marked improvements in psychological distress and quality of life in a real-world clinical setting, according to an ambispective observational cohort study conducted in routine dermatology practice. The findings highlight benefits extending beyond skin clearance.

The study included 133 patients treated with biologics at a referral center in Northern Italy. Baseline clinical and psychological data were extracted from medical records, and follow-up assessments were performed at 6 months. Measures included the Psoriasis Area and Severity Index (PASI), Patient Health Questionnaire-9 (PHQ-9), Brief Symptom Inventory, Perceived Stress Scale, Dermatology Life Quality Index (DLQI), and World Health Organization Quality of Life Scale.

At follow up, significant reductions in depressive symptoms were observed. PHQ-9 scores decreased (P < 0.001), with moderate-to-severe depression prevalence declining from 28.6% at baseline to 5.3% at 6 months. Substantial perceived stress dropped from 15.0% to 1.5%. Dermatology-specific quality of life improved markedly, with 89.5% of patients reporting minimal impact (DLQI 0–1) at follow up.

Skin outcomes were robust, with 77.4% achieving complete clearance (PASI 100). However, regression analyses demonstrated that baseline psychological symptoms were the strongest predictors of follow-up psychological and quality of life outcomes. Additional predictors included baseline PASI, female sex, psychiatric comorbidity, and prior biologic exposure.

The authors concluded that “biologic therapies confer multidimensional benefits in moderate-to-severe psoriasis, extending beyond skin clearance to substantial reductions in psychological distress and improvements in QoL [quality of life].” They further emphasized that these findings “support a patient-centered model integrating dermatologic and mental health care.”
